A Senior Navigator is someone who walks alongside you through the healthcare journey. Think of it as having a knowledgeable, calm friend who helps you stay organized, understand what your doctor said, and never feel alone in the process.
This is not a medical service. A Senior Navigator does not diagnose, treat, or prescribe. What we do is help you feel prepared, heard, and supported every step of the way.

Alexis “Lex” Harper-Jones is the founder of Collective Roots LLC and your Senior Navigator. Lex has a Master’s degree in Counseling and over 20 years of experience in behavioral health, care coordination, and supporting people through life transitions.
Lex is calm, organized, and easy to talk to. She listens carefully, asks thoughtful questions on your behalf, and makes sure you leave every appointment feeling clear about what was said and what comes next.

Ideal for important appointments, specialist visits, procedures, or second opinions. Includes pre-appointment preparation, in-person or virtual attendance, detailed notes, provider instruction summaries, post-appointment follow-up, and family communication support.
Ongoing structured support for seniors and families who need continuity, organization, and proactive wellness coordination. Includes multiple appointments per month, wellness tracking, care coordination follow-up, family updates, secure documentation, and medication reminder support.
Designed for clients managing complex healthcare situations with multiple providers. Support areas include oncology, neurology, memory care, post-surgical recovery, chronic illness, and VA-related healthcare navigation.
Custom support plans are always available. We will take the time to understand your needs and build something that works for you.

Senior Navigator services are non-medical and non-clinical. Lex is not a doctor, nurse, therapist, or case manager, and these services do not replace your medical care team. All personal information is handled with full privacy, professionalism, and care.

A home health aide provides hands-on physical care such as bathing, dressing, and medication administration. A Senior Navigator does not provide any hands-on or medical care. What I do is organize, coordinate, advocate, and support. I attend appointments, take notes, help you understand what was said, manage your care documents, and keep your family informed. Think of it as having a calm, organized guide alongside you — not a caregiver.
Both options are available depending on the appointment and your needs. In-person attendance is available for clients in the Maryland, Virginia, and Washington D.C. area. Virtual attendance via phone or video is available for clients outside that area or for telehealth appointments. We will discuss what works best for your situation during your free consultation.
Pricing is based on the level and frequency of support you need. I offer Appointment Navigation Support for one-time or occasional visits, Monthly Navigation Support for ongoing coordination, and Specialty Care Navigation for complex care situations. Custom plans are always available. Pricing details are discussed during your free consultation so we can build something that fits your needs and budget.
At this time, Senior Navigator services are not covered by insurance or Medicare, as they are non-clinical and non-medical in nature. Some clients use health savings accounts (HSA) or flexible spending accounts (FSA) to cover wellness coordination services — check with your plan administrator to see if you qualify. We are happy to provide documentation of services upon request.
In-person services are currently available in the Maryland, Virginia, and Washington D.C. area. Virtual navigation support — including appointment attendance, care coordination, family communication, and document organization — is available to clients anywhere. If you are unsure whether your location is covered, reach out and we will figure it out together.
Absolutely. All client information — including health details, appointment notes, provider contacts, and family communication — is handled with full confidentiality, professionalism, and care. Your information is never shared with third parties. Privacy and dignity are at the core of everything we do at Collective Roots LLC.

From there, you decide if it feels like the right fit.
Yes — and many families find this to be one of the most valuable parts of the service. I can provide regular updates to designated family members after appointments, help facilitate communication between family and providers, and serve as a consistent point of contact for families supporting a loved one from a distance. Family involvement is always tailored to what the client is comfortable with.
